John William Fazio, Jr., 74 of Washington, WV, passed away peacefully at home surrounded by his family on April 21, 2026, gaining his heavenly wings. John was born on March 22, 1952, in Wheeling, WV, a son of the late John William Fazio Sr. and Ida Morris Fazio, both of Wheeling, WV.
John graduated from Triadelphia High School in 1970. From there, he followed in his parents’ footsteps to become the General Manager of Elm Grove Lanes, working for AMF for 35 years. John was also a PBA member and ended up in Washington, WV, to finish his career with Emerson Lanes, where he served as the General Manager for 20 years, retiring in December 2023.
Some of John’s fond memories were sharing his love of baseball with his four children, coaching little league baseball, and then serving as the head coach at Bishop Donahue High School for four years.
John is survived by his wife Theresa (Terri) Fazio of 49 years, his twin daughters’ Alexis Fazio (Washington, WV), and Jennifer Fazio Ebert (Harry) of Canal Winchester, Ohio, his sons Nathan Fazio (Kirby) of Wheeling, and Jason Fazio (Hayley) of Grove City, Ohio and his eight grandchildren, Garrett, Cruz, Canaan, Creed, Jackson, Knox, Sylas, and Oaklyn.
Also survived by his brother-in-law, Gary Hamilton (Mary) of Wheeling, sisters-in-law Paula Teufel, Heidi Hamilton, and Karen Hamilton of Wheeling, and several nieces and nephews.
In addition to his parents, he was preceded in death by his in-laws, Paul and Eleanor Hamilton, brother-in-law Jeffrey Hamilton, and sister-in-law Susanne Hamilton.
The family extends their heartfelt gratitude to the oncology care teams of Memorial Health System, Kelli Cawley, MD, Rajendra Bhati, MD, Douglas Keyser, MD, Marcia McCullough, NP, and the many nurses and social workers, for their dedication, empathy, compassion, and support. They truly were a blessing through his cancer journey.
